Lifetime Membership Registration (Individual/Family)

2. Start by entering your Email Address below, the form will continue.
3. When it is complete, please submit and make payment.
   (Regular Lifetime membership is $650 for Individual (1 member) or Family Membership (up to 2 members, please specify "family" if the membership is for 2 family members, MUST live in the same household)

Your membership will become active after BOTH registration and payment are received. There is no expiration for a Lifetime membership.

**PLEASE NOTE: Your membership will not be activated without a completed registration form AND the completed payment. (you will receive a receipt confirmation when payment is complete. If you do not receive this, your payment did not go through!)

***Amateur Passes made without an active membership will NOT count toward Qualification of the Master Amateur Invitational.***
