Fill out the Annual registration form below. When it is complete, please submit and continue to make payment.
(2019-2020 Annual membership is $50 for Individual (1 member) or Family Membership (up to 2 members, must live in the same household) specify below)
The 2019-2020 Qualifying year begins March 1, 2019 and goes through February 29, 2020. Your membership will be active after registration and payment are both received, through February 29, 2020.
**PLEASE NOTE: Your membership will not be activated without a completed registration form AND the completed payment. (you will receive a receipt confirmation of payment when complete. If you do not receive this, your payment did not go through! Contact us if you need assistance)

***Amateur Passes made without an active membership will NOT count toward Qualification of the 2020 MAI.
